    I went to see my physician in order to have WCI form no. 3 filled (Initial medical examination findings). 

    However my physician has given me a form which needs to be filled beforehand by my manager at work.

    Upon presenting this form at work, they absolutely refused to cooperate and fill it for me.

    Is there something I can do in order to force my manager at work to cooperate and fill the form which my physician has given me?
