    To Submit a Claim for Medical Care services for an Injury or Disease
    When there is a new injury or disease occurred due to work-related injury or disease when he/she is under medical care.

    • When the worker needs to undergo medical care due to an additional disease.
    • When the worker needs to undergo medical care due to a new disease cause by any existing work-related injuries or diseases.
